- You should receive your new medical ID card(s) from UHC by the end of December. Employees who cover dependents will receive two cards in total. Be sure to replace your Aetna or Humana card with the UHC card when it arrives and give it to your providers the next time you go for a visit. If you don’t receive your UHC medical plan ID card this month or you need an additional copy for a family member, you can print a card online or use the mobile app to view, fax or email your ID card. - No need for vision ID cards. UHC vision providers don’t require ID cards because providers can verify your coverage with your name and date of birth. However, if you or a family member would prefer to have one, read UnitedHealthcare Medical and Vision ID Cards for more information.
- Last chance to earn up to $450 off your 2016 medical premiums. If you’re participating in the Health Management program, be sure to get your health screening, complete the Health Risk Questionnaire (HRQ) and/or finish your Take Action activity by Dec. 31. Look for information on what is new for the 2016 Health Management program early next year.
- Want to receive your W-2 electronically? Visit EmployeeConnect no later than Jan. 15. Click the “Your Pay” tab and go to “Taxes and Deductions” to consent to the electronic version. (If you have previously registered, you do not need to register again.)
- UHC will take over administration of the Health Reimbursement Account (HRA). Humana PCA cards expire Dec. 31. Your current administrator, Aetna or Humana, will use your 2015 balance to pay any remaining claims for 2015. After 2015, your current administrator will send a report to UHC and transition your funds. Those funds will be available to you by the beginning of April.
- Current Humana members who enroll in the HRA for 2016 won’t have a debit card next year. Instead, they’ll be reimbursed automatically for any medical claim balances. Humana HRA debit cards will not work after Dec. 31, 2015.
- HealthEquity debit cards are valid for three years. If you received a debit card from HealthEquity in 2015, you should continue to use that card to pay for eligible expenses. If you are new to the Health Savings Account (HSA) option, you will receive your HSA debit card by the end of December.
- The Rockwell Automation Service Center and the Rockwell Automation Service Center at Fidelity will be closed on Christmas (Dec. 25) and New Year’s Day (Jan. 1).
