  • Have you checked your doctor’s status lately? Now is a good time to verify whether your physician is designated a Tier 1 primary care provider or specialist and/or considered in-network for 2017. Health care providers move in and out of these networks on occasion, and Tier 1 designation can change annually, so it’s good to double check. To do so, search the Choice Plus network for your provider, and check for the Tier 1 and in-network icons next to his/her name.
  • Reminder: You don’t need a dental plan ID card. If you’re enrolled in a Rockwell Automation dental option, simply provide your 10-digit employee ID number at your appointment. (Your employee ID serves as your MetLife dental ID number.) If you prefer to have a card, you can print one from the MetLife website. Just be sure to add your employee ID number.
