You should have received the 2019 Take Action guide in your home mail, but you can also download it here. It’s got everything you need to know from how to sign up to learning which activity choices might be right for you.

New activities added for 2019

We’ve added two new activities for 2019: Real Appeal and Aware.

Real Appeal: You can participate in this 12-week weight loss program that includes weight tracking, coaching and group support if you’re enrolled in a UnitedHealthcare medical plan.

Aware: This six-session mindfulness-based phone coaching program connects you with a professional to help you reduce your stress through self-guided practice.

See the 2019 Take Action guide for more details.

Don’t stop there

Take Action is only one part of the three-part Health Management Program. By completing the other two parts of the program—the Health Risk Questionnaire (HRQ) and health screening— before the end of the year, you can earn an additional $250 off your 2020 medical plan premiums.
